Due to recent changes to HMSA travel requirements, we now need to collect additional information, such as your ID, to secure travel insurance approval. This may include your weight, which is sometimes required for smaller flights. Thanks for your cooperation!

Please submit your travel information AT LEAST 7 DAYS PRIOR to your appointment and allow 2–3 business days for processing and to meet insurance requirements. If your appointment is urgent or involves surgery, an exception to the 7-day requirement may be considered; otherwise, please be prepared to reschedule.
