New patient process for patients whose last primary care provider did NOT use EPIC electronic medical record
(If you are not sure if your last provider used EPIC, please call them and ask, as this will make your new patient process much easier. Wilcox and Kapiolani Medical Center both use EPIC. )
Complete the ‘Patient Registration Form.’ Be sure put your personalized acceptance code that you received from our office on the top of the form.
Complete the ‘Patient History Form.’
Complete the ‘Authorization to Release Protected Health Information.’ Send it to your last physician and obtain the patient’s previous medical records.
**If your child has no complex medical problems, then we will only need the following three items: ☐Vaccination Records ☐Growth Charts ☐Last Well Check Record
**If your child has complex medical problems, then you will need to obtain the entire medical record.
If you are uncertain of what information we will need then please call us and we can help you figure out what records will be ideal.
Once you receive their medical records, you are finished! Please email, fax, or deliver all of the following to our office.

Insurance Card of Patient (copy of front and back)
